Employee Eligibility Requirements
Employee must be employed in a permanent and full-time basis working 30 or more hours per week.The employee must complete the insurance benefit election forms with 31 days of the date of employment or status change.
Dependent Eligibility Requirement
Your lawful spouse, and dependent children younger than age 26 are eligible to enroll in benefits.

March 1 - April 30
Effective Date
Coverage begins the first of the month following 30 days of continuous employment.
